Team Conference


Team Conference

Effective 1/1/08 Medical  Team Conference with direct (face-to-face) contact with patient and /or family by a physician  has been deleted.   You may report using (E&M) codes using time as the key controlling factor for code selection when counseling and/or coordination of care dominates the service

  • Include face to face participation by a minimum of three qualified healthcare professionals from different specialties (each of whom provide direct care to the patient).
  • Participate is actively involved in the development, revision, coordination and implementation of healthcare services needed by the patient.
  • Reporting participants shall have performed face to face evaluations or treatments of the patient, independent of any care team conference, within the past 60 days.
  • Reporting participants shall document their participation in the team conference as well as their contributed information and subsequent treatment recommendations.
  • Only one individual from the same speciality may report team conference at the same encounter.
  • Time starts at the beginning of the review of an individual patient and ends at the conclusion of the review.
  • The reporting participant shall be present for all time reported.
  • Time is not limited to the time that the participant is communicating to other team members or patient and/or family members.
  • Time reported for medical team conferences may not be used in the determination of time for other services such as care plan oversight, prolonged services, psychotherapy or any E/M service. 
  • Team conference services of less than 30 minutes duration are not reported separately.
  • Code determined based on whether the visit is with or without direct face-to-face contact with patient and/or family and the credentials of billing provider.
